At Shift, we’re here to guide you on your SDA housing journey, for many people this will start with eligibility. The NDIS will assess your eligibility for SDA against their criteria and will start with asking the following questions:

  • 1 Do you have extreme functional impairment or very high support needs?
  • 2 Do you have specialist disability accommodation needs requirements?
  • 3 Does specialist disability accommodation meet the NDIS funding criteria for you?

They may also ask for additional supporting evidence such as reports. An Occupational Therapist can complete a SDA Functional Assessment and report for you. For more information on eligibility click here.

WHAT DO I NEED TO HAVE IN MY NDIS PLAN TO EXPLORE SDA?

To help you in your journey to receive SDA funding from the NDIS, we suggest you have the following in your NDIS plan:

  • 1 A goal that relates to exploring your accommodation needs. If you do not have an accommodation goal, you can complete a Home and Living Supports request form or request a Change of Circumstances to review your goals
  • 2 20 hours of capacity building funding, for an Occupational Therapist to complete an SDA Functional Assessment and report
  • 3 Funding for a Support Coordinator, who will assist you to explore your accommodation options, write a housing report, and coordinate your SDA application to NDIS

WHAT DO I NEED TO DO IF I WANT TO EXPLORE SDA OPTIONS FOR ME?

Reach out to Rocky Bay Referrals and Transitions Managers Emma and Michele or connect with a Support Coordinator who can assist you to navigate the process of requesting an OT Functional Assessment for SDA. This assessment and report will provide evidence in support of your eligibility for SDA.

How it works

At SHIFT, we’re here to guide you on your SDA housing journey, below if a snapshot of what you journey could look like.

  • 1 To start your SDA journey, reach out to us at Shift, or meet with your Support Coordinator to discuss housing options
  • 2 Complete a SDA Functional Assessment with an Occupational Therapist. This report, along with any other supporting evidence, are submitted along with your SDA Housing Plan to the NDIA
  • 3 If approved, you will be notified of your funding level (i.e. Improved Liveability, Fully Accessible, High Physical Supports, and Robust), design category, location and number of occupants. You may also be eligible for SIL funding (SDA funding is for the physical property only)
  • 4 Upon SDA approval, you can partner with Shift to select your new home based on your requirements
  • 5 Congratulations! You can now move into your new home, and start accessing your support services
